After completing online registration using the form below, please upload your student's immunization records, birth certificate, behavior report, transcript (grades 8–12), and vision screening (Kindergarten).
State law requires that we view the original birth certificate. Please bring it to the school at your earliest convenience.
Students who do not submit all required registration materials by the stated deadline may be denied enrollment.
